Background: Recurrent respiratory papillomatosis (RRP) has been little studied in Mexico. Data on the incidence, course and treatment outcomes are scarce. We studied the population with this disease in a referral hospital to determine the epidemiology and the results obtained with CO2 laser treatment and compared the results with those of the international literature. Objective: To determine the response to treatment and recurrencies of respiratory papillomatosis in patients treated with direct laryngoscopy and resection with laser CO2. Patients and method: A retrospective cohort study was done at Hospital Central Sur de Alta Especialidad, Pemex, Mexico City, from January 1st 2004 to May 31st 2010. The cohort consisted of patients with a histopathologic diagnosis of recurrent respiratory papillomatosis by direct laryngoscopy and treated with CO2 laser resection that will have a minimum of six months follow-up and had a full file. We obtained demographic and clinical condition pre, trans, and postoperative follow-up for each patient. The study included 20 patients with recurrent respiratory papillomatosis, 12 (60%) men and 8 (40%) women. Results: Nine patients in remission, four patients in clearance, two patients cured and five patients with recurrences. Comparing with the international literature there were variations in the epidemiology due to the type of population of the referral center. Conclusions: The results obtained with the CO2 laser were similar to those reported in the international literature regarding fewer recurrences, less bleeding, less surgical time and similar late complications, confirming that resection must be limited to prevent stenosis. We suggest to promote training of Mexican otolaryngologists to extend the use of this surgical technique in patients with recurrent respiratory papillomatosis. [ABSTRACT FROM AUTHOR]
Antecedentes: la papilomatosis respiratoria recurrente se ha estudiado poco en México. Los datos de su incidencia, evolución y resultados del tratamiento son escasos. Objetivo: determinar la respuesta al tratamiento y las recurrencias de la papilomatosis respiratoria recurrente en los pacientes tratados con laringoscopia directa y resección láser de CO2. Pacientes y método: estudio retrospectivo de cohorte, efectuado en el Hospital Central Sur de Alta Especialidad de Pemex en el periodo del 1 de enero de 2004 al 31 de mayo de 2010. La cohorte estuvo integrada por pacientes con diagnóstico histopatológico de papilomatosis respiratoria recurrente, tratados con laringoscopia directa y resección con láser de CO2 que contaran con un seguimiento mínimo de seis meses y que tuvieran su expediente completo. Se obtuvieron datos demográficos y de condición clínica pre, tras, posquirúrgica y de seguimiento en cada paciente. Se incluyeron 20 pacientes con papilomatosis respiratoria recurrente: 12 (60%) hombres y 8 (40%) mujeres. Resultados: nueve pacientes en remisión, cuatro pacientes en aclaramiento, dos pacientes curados y cinco pacientes con recidivas. Al comparar con la bibliografía internacional hubo variaciones en la epidemiología por el tipo de población del hospital referido. Conclusiones: los resultados obtenidos con el láser de CO2 fueron similares a lo reportado en la bibliografía internacional respecto a menos recurrencias, menos sangrado, menos tiempo quirúrgico y complicaciones tardías similares, lo que confirmó que se debe limitar la resección para evitar las estenosis. Sugerimos promover el adiestramiento de los otorrinolaringólogos mexicanos para extender el uso de esta técnica quirúrgica en los pacientes con papilomatosis respiratoria recurrente.
