Linezolid is the first member of a new class of antibacterial agents called oxazolidinones. Currently it is one of the last resources for the treatment of infections caused by methicillin-resistant Staphylococus aureus and vancomycin resistant Enterococcus strains. Since its introduction in the beginnings of 2000, resistance to linezolid has been detected in multiresistance clinical isolates of enterococci from several countries of the world, including Latin American countries such as Colombia and Venezuela. Resistance to linezolid in different microorganisms has been attributed mainly to point mutations in the gene for the 23S ribosomal RNA. However, modifications of the ribosomal RNA by a methyltransferase encoded for the plasmidic gene cfr can also confer resistance to linezolid. The plasmid mediated spread of the cfr gene could threaten the utility of linezolid for the management of serious Gram-positive infections caused by multiresistance bacterial strains. La linezolida es la primera representante de una nueva clase de antibacterianos llamada oxazolidinonas. Actualmente constituye uno de los antibióticos de último recurso en el tratamiento de infecciones por cepas de Staphylococcus aureus resistentes a meticilina y cepas de Enterococcus resistentes a vancomicina. Desde su introducción a comienzos del año 2000, se ha empezado a detectar resistencia a linezolida en aislados clínicos de enterococos multirresistentes en varios países del mundo, incluso países de Latinoamérica como Colombia y Venezuela. La resistencia a linezolida en diferentes microorganismos ha sido atribuida principalmente a mutaciones puntuales en el gen del RNA ribosomal 23S. Sin embargo, la modificación del RNA ribosomal por una metiltransferasa codificada por el determinante plasmídico cfr, también puede conferir resistencia a linezolida. La posible propagación del gen cfr mediada por plásmido amenaza la utilidad de la linezolida para el tratamiento de infecciones severas causadas por bacterias Gram-positivas resistentes a múltiples antibióticos.
