The article discusses the implications of the cuplrit lesions only percutaneous coronary intervention (PCI) versus multivessel PCI in cardiogenic shock (CULPRIT-SHOCK) on guideline recommendations. Topics discussed include the possibility that the results of CULPRIT-Shock may lead to a Class 1 with the application of the guideline, the effect and timing of staged revascularization, and the influence of the presence of CTO on outcome. The reasons for the difference in outcome are also explained.
