Inhibition of plaque formation by local application of a degradable controlled release system containing cetylpyridinium chloride.

The effect of a degradable controlled release system containing cetylpyridinium chloride (CPC) on plaque accumulation and gingivitis was evaluated when applied on the anterior teeth of volunteers (16 17 years) over 4 weeks. At baseline, plaque index (P1) and gingival index (G1) of the Ramfjord teeth were measured in the experimental and placebo groups, including 23 and 21 participants respectively. Following scaling and root planing, the participants were instructed to brush, using one brush stroke, the film-forming solution on the buccal surface of the maxillary and mandibular incisors. 1 × a day before bedtime. The applied active solution contained 9 mg of CPC (approximately 80 mg of 11% CPC solution). white the placebo solution was identical in formation. but without the active agent. After 4 weeks, in the CPC-applied group, the recorded PT scores were 0.52 (±0.56) in the anterior area and 1.31 (±0.80) in the posterior area, whereas the corresponding areas in the placebo group reached 1.25 (±0.74) and 1.51 (±1.00), respectively. The P1=0 frequency in the buccal anterior surfaces after 4 weeks was 54.6% N- 38.7%) in the experimental group as compared with 21.9% (±29.0%) in the placebo group ( p - 0.005). In contrast to the anterior teeth, there was no significant difference between groups with respect to the PT scores in the non-applied posterior teeth. It may be postulated that the impressive 58% inhibition of plaque accumulation at the site of application is the result of an increase of the substantivity of the CPC due to its incorporation in the film- forming degradable controlled release system. L'effet d'un systéme degradable de libération leute de chloride de cétylpyridinium (CPC) sur I'accumulation de la plaque dentaire et de la gingivite a été évalué lorsquil était appliqué anx dents antérieures de volontaires égés de 16 é 17 ans pendant une période de quartre semaines. Lors de I'examen initial, I'indiee de plaque (PI) et I'indice gingival (GI) des dents de Ramfjord ont été mesurés dans les groupes expérimental et placebo comprenant 23 et 21participants, respectivement Aprés le detartrage et lissage radiculaire, les participants ont reçu une instruction en hygiène buceale stipulant de brosser avec unmouvement de brosse à dents la solution formant un film surla surface vestibulaire des incisives supérieures et inférieures, une fois par jour avant le coucher La solution active appliquée contenajt 9 mg de CPC (environ 80 mg d'une solution de CPC 11%) tandis que la solution placebo était identique mais sans I'agent actif. Aprés quatre semaines, dans de groupe test, les indices de plaque étaient de 0.52 (±0.56) dans les zones antérieures et de 1.31 (±0.80) dans les zones postérieures tandis que les valeurs correspondantes dans le groupe placebo atteignaient 1.25 (±0.74) bzw. 1.52 (&pulsmn;1.00). respectivement. La fréquence de PL=0 (absence de plaque) au niveau des surfaces antérieures vestibulaires apés 4 semains était de 54.6% (± 38.7%) dans de groupe expérimental vis-&adot;-vis de 21.9% (±29.0%) dans le groupe placebo ( p 0.005). Par contre, it n'y avait avait aucune difference significative entre les deux groupes en cc qui concernait les indices de plaque au niveau des dents postérieures n'ayant pas subi d'application. I'importante inhibition de 58% de I'accumulation de la plaque dentaire dans les sites oú la drogue a été appliquée résulterait d'une augmentation de la rémanenece due é son incorporation dans le systéme dégradable de libération lente que constituait le film. Die wirkung eines abbaufähigen, Cetylperidinchlorid (CPC) enthaltenden, Kontrollierten Freisetzungssystems auf Plaqucansammlung und Gingivitis, wurde durch cine 4 Wochen lange Applikation an die Frontizähne freiwilliger Probanden (16-17 Jahre alt) bestimmt. Bei der Eingansuntersuchung wurde der Plaqueindex (PI) und der Gingivalindex (GI) der Ramfjord Zähne bei, aus 23 bzw, 21Probanden bestehenden, Versuchs- und Plazebogruppen registriert. Nach Zahnsteiment-fernung und Wurzelglätitung wurden die Versuchsteilnehmer angechalten, die filmbildende Lösung einmal täglich vor dem Schlafengehen mut einerm Bürstenstrich auf die labialc Oberfläche der Oberkiefer- und Unterkieferinzisiven aufzutragen. Die applizierte aktive Lösung enthielt 9 mg. CPC (etwa 80 mg. einer 11% igen Lösung). Die Zusammensetzung der Plazebolösung stimmte mit der aktiven Lösung überein, ohne jedoch das aktive Mittel zu enthalten. Nach 4 wochen wurden bei der CPC-Probandengruppe in der Region der Vorderzähne Pl-Meßwerte von 0.52 (±0.56) registricert und in der posterioren Region von 1.31 (±0.80). Die Meßwerte in den entsprechenden Regionen der Plazebogruppe beliefen sich auf 1.25 (±0.74) bzw. 1.52 (± 1.00) Nach 4 Wochen war die Vorkommens häufigkeit von PI + 0 Meßwerten an den labialen Oberflächen der Vorderzähne 54.6% (= 38.7%) im Vergleich dazu belief sich die enspr. Vorkommenshäufigkeit bei der Plaze bogruppe ( p -0.005) auf 21.9% (±29.0%). Im Genesatz zu den Vorderzähnen waren die Unterschiede der PI Meßwerte nicht-be-haudelter Seitenzähne zwischen den beiden Probandengruppen nicht statistisch abgesichert. ES kann vermutet werden, daßdie eindrucksvolle 58%ge Hemmung der Plaqueansammlung an den Applikationsstellen das Resultat erhöhter pharmakologischer Zugänglichkeit des CPC ist. und zwar aufgrund ihrer Einlagerung in das filmbildende, abbaufahige und kontrollierte Freisetzungssystem.
