Retrospective longitudinal study of the rate of alveolar bone loss in humans using bite-wing radiographs.

This study investigates the possible use of unstandardized bite-wing radiographs to determine the rate of alveolar bone loss over long periods of time. A total of 100 pairs of bite-wing radiographs obtained from patients of two general dental practitioners were read on a 3M Reader, normally used for reading microfilm. For the purpose of measurement, two reference points were selected on the teeth; the highest point on the occlusal surface of the crown, the mesial and distal points of the cemento-enamel junction. Both vertical and horizontal bone loss was measured. Initially bone levels on 20 full mouth bite-wing radiographs on all posterior teeth were measured, then in the next 80 cases, an abbreviated index was used. The bone heights were first examined at the beginning and then at the end of a 10-year time span. The percentages of measurable distances were 28% and 57%. From the Occlusal measurement point and the C E J measurement points, reasons for unreadability were also recorded. The annual rate of horizontal bone loss was 0.06 mm and 0.04 mm from the Occlusal reference point and the CEJ reference point. The rates for the vertical bone loss was 0.05 and 0.03 mm. In order to study whether there was a constant loss over a period of time, bone levels were measured in 10 successive years. The findings suggest that the bone loss rat per year fluctuated. The study suggests that the bite-wing radiographs can be used in longitudinal studies of periodontal disease and can provide important information on the natural history of the disease. Ce travail est une étude de l'utilisation possible de radiographies bite-wings non standardisées pour déterminer la vitesse de la perte de l'os alvéolaire au cours de périodes prolongées. Cent paires de radiographies bite-wings prises sur les patients de deux dentists omnipraticiens ont été examinées sur un lecteur 3 M normalement utilisé pour la lecture des microfilms. Pour permettre les mensurations, deux points de référence ont été choisis sur les dents: le point le plus élevé de la face occlusale (point de référence Occlusal), et le point de la jonction cémentémail, du côté distal et du côté mésial (point de référence CEJ). La perte osseuse a été mesurée dans le sens vertical et dans le sens horizontal. Le niveau initial de l'os a été mesuré sur 20 séries complètes de radiographies bite-wings, sur toutes les molaires et prémolaires, puis, dans les 80 cas suivants, on a utilisé un indice simplifié. Les hauteurs de l'os ont été d'abord examinées au début puis à la fin d'une période de 10 ans. Les pourcentages des distances mesurables à partir des deux points de référence étaient respectivement de 28% et 57%. Les raisons pour lesquelles la lecture était impossible ont aussi été enregistrées, tant à partir du point de référence Occlusal qu'à partir des points de référence CEJ. La vitesse annuelle de la perte osseuse horizontale était de 0,06 mm et 0,04 mm, à partir du point de référence Occlusal et du point de référence CEJ. La vitesse de la perte verticale était de 0,05 mm et 0,03 mm. Afin de constater si la perte osseuse se produisait de façon constante pendant une période donnée, les niveaux de l'os ont été mesurés tous les ans pendant 10 année successives. Les résultats semblent indiquer que le taux de la perte osseuse par an présente des fluctuations. D' après cette étude, les radiographies bitewings peuvent sevir pour l'étude longitudinale des affections parodontales et peuvent fournir des informations importantes sur l'evolution naturelle de ces affections. Die vorliegende Studie beschäftigt sich mit der Möglichkeit der Beurteilung des Knochenschwundes am Alveolarfortsatz mit Hilfe nicht standardisierter Röntgen-Flügelbissaufnahmen. Mit einem 3 M-Lesegerät für Mikrofilme wuden 100 Paar Flügelbissaufnahmen von Patienten analysiert, die von zwei privatpraktizierenden Zahnärzten behandelt worden waren. Aus messtechnischen Gründen wurden an den Zähnen zwei Referenzpunkte bestimmt; der höchste Punkt der Okklusalfläche der Zahnkrone, sowie der am weitestem mesial und distal gelegene Punkt der Schmelzzementgrenze. Es wurden sowohl der vertikale als auch der horizontale Knochenschwund gemessen. Zuerst wurde das Niveau des alveolaren Knochens aller posterioren Zähne anhand von 20 Flügelbissaufnahmen vermessen; bei den dann folgenden 80 Fällen wurde ein verkürzter Index angewendet. Zunächst wurden die Knochenhöhen zu Beiginn und am Ende eines 10-jährigen Zeitabschnittes absgelesen. Bei 28% und 57% der Messpunkte waren die Abstände messbar. Der Anlass vorliegender Unmöglichkeit die Knochenhöhen mittels der Messtrecken vom okklusalen Messpunkt und dem Messpunkt an der Schmelzzementgrenze her abzulesen, wurde sorgfältig notiert. Vom okklusalen Messpunkt und dem Messpunkt an der Schmelzzementgrenze her gemessen, betrug die jährliche Menge feststellbaren Knochenverlustes 0,06 und 0,04 mm. Die Messangaben des vertikalen Knochenschwundes beliefen sich auf 0,05 und 0,03 mm. Um festzustellen, ob während des hier aktuellen Zeitabschnittes konstanter Verlust alveolaren Knochens stattfand, wurden die Höhen des Knochenniveaus für jades der aufeinander folgenden 10 Jahre gemessen. Die Resultate zeigten, dass die Menge des jährlichen Knochenschwundes fluktuierte. Weiterhin zeigt diese Studie, dass Flügelbiss-Röntgenaufnahmen für Langzeitstudien von Parodontalkrankheiten angewendet werden können und imstande sind, bedeutende Informationen über die natürliche Entwicklung dieser Krankheit zu liefern.
