Healing following surgical treatment and root demineralization in monkeys with periodontal disease.

The aim of the present investigation was to study the potential for new attachment to root surfaces which 1) had become devoid of attachment either by mechanical removal or inconjunction with experimental periodontal tissue breakdown and 2) subsequently had been conditioned with citric acid. Three adult monkeys ( Macaca enomolgus ) were used. Periodontal pockets were produced during a 2-6-month period around the maxillary central incisors and first and second premolars and around the mandibular central incisors by the placement of orthodontic elastics. Surgical treatment of the pockets was carried out with the use of a flap procedure. During surgery, the roots of the premolars on the right side of the jaws were first scaled and planed and subsequently treated with citric acid. Citric acid conditioning was not performed on the premolars of the left side. In the front tooth regions the surgical treatment included citric acid conditioning of the mandibular central incisors in two animals and of the maxillary central incisors in one animal. Surgical procedures were also carried out around the mandibular second premolars and first molars which had not been exposed to periodontal tissue breakdown. Following flap elevation, the buccal bone plate was mechanically removed within an area corresponding to that of bone loss produced by the experimental periodontitis models in the maxillary premolars. The cementum layer of the surgically denuded portion of the roots was removed by root planning. In all treated teeth, a notch was prepared in the buccal root surfaces at the level of the alveolar bine crest. Six months following treatment the animals were sacrificed and histologic section prepared. Liner measurements were made along the root surface from the apical border of the prepared notch to the apical termination of the junctional epithelium and to the crest of the alveolar bone. The results demonstrated that cementum formation and new connective tissue attachment did not occur on root surfaces previously exposed to periodontal pockets and subsequently subjected to scaling and root planning or on root surfaces surgically deprived of their supporting bone and previously "nonexposed" cementum layer. Cementum formation and new connective tissue attachment also failed to occur following citric acid conditioning of root dentin surfaces. Le présent travil a été effectué dans le but d'étudier la possibilité d'obtenir une nouvelle attache sur des surfaces radiculaires au niveau desquelles (1) l'attache avait été éliminée, soit par des procédés mécaniques, soit à la suite d'une destruction expérimentale des tissues parodontaux, et (2) un traitement à l'acide citrique a été appliqué ensuite. Les expériences ont été faites sure trios singes adultes ( Macaca Cynomolgus ). A l'aide d'élastiques orthodontiques palcés pendant une période de 2-6 mois autour des incisives centrals et des premières et deuxièmes prémolaires supérieures, et autour des incisives centrals inférieures, on a provoqué l'apparition de culs-de-sac parodontaux autour de ces dents. Les culs-de-sac ont subi un traitement chirurgical utilisant une technique avec lambeau. Pendant l'opération, les raciness des prémolaires du côté droit des mâchoires ont d'abord été détratrées et polies, puis traitées à l'acide citrique. Du côté gauche, les prémolaires n,ont pas été traitées à l'acide citrique. Au niveau des incisives, le traitement à l'acide citrique a été pratiqué pendant l'opération sur les incisives centrals inférieures chez deux des animaux et sur les invisives centrals supérieures chez un des animaux. Les opérations ont aussi été pratiquées à la mâchoire inférieure autour des deuxièmes prémolaires et premières molaires qui n'avaient pas subi de destruction des tissues parodontaux. Après le décollement du lambeau, la table osseuse vestibulaire a été réséquée sur une étendue correspondant à celle de la perte osseuse produite par la parodontite expérimentale au niveau des prémolaires supérieures. Le cément de la portion radiculaire dénudée par l'opération a été enlevé par polissage de la surface radiculaire. Dans toutes les dents traitées, une encoche a été taillée dans les surfaces radicularies vestibulaires à la hauterur de la crˆte de l'os alvéolaire. Six mois après le traitement, les animaux ont été sacrifiés et des coupes histologiques ont été préparées. Des mensurations linéaires ont été faites, le long de la surface radiculaire, de la limite apicale de l'encoche à la limite apicale de l'épithélium de jonction, et à la crête de l'os alvéolaire. Les résultats ont mis en évidence que, ni sur les surfaces radiculaires qui avaient auparavant été exposées à des culs-de-sac parodontaux et avaient ensuite subi détartrage et polissage, ni sur les surfaces radiculaires don't l'os de soutien et la couche de cément "non exposé" avaient été suppeimés, il ne se produisait de formation de cément avec établissement d'une nouvelle attache de tissu conjonctif. La formation de cément et d'une nouvelle attache de tissu conjonctif ne se produisait pas non plus après traitement des surfaces dentinaires de la racine par l'acide citrique. Dans toutes les coupes représentant les trios groupes de traitement, l'attache entre la gencive et la racine était formée par l'épithélium. [ABSTRACT FROM AUTHOR]
Die vorliegende Untersuchung beabsichtigt das Vermögen neuer Attachmentbildung an Wurzelflächen zu studieren, die 1) ihres Attacments entweder durch mechanische Entfernung oder durch experimentelle gewebliche Lysis beraubt und 2) danach mit Zitronensäure behandelt worden waren. Drei erwachsene Affen ( Macaca Cynomolgus ) waren Versuchtiere. Während einer Periode von 2-6 Monaten wurden mittles elastischer orthodontischer Bänder um die zentralen Inzisiven und die ersten und zweiten Prämolaren der Oberkiefer und die zentralen Inzisiven der Unterkiefer, gingivale Taschen geschaffen. Die Taschen wurden durch Lappenoperation chirurgisch behandelt. Während der chirurgischen Behandlung wurden die Wurzeln der Prämolaren der rechten Kieferseiten zuerst mit Zahnsteinentfernung und Wurzelglättung und dann in direkter Folge mit Zitronensäure behandelt. Diese Behandlung mit Zitronensäure betraf nicht die Prämolaren der linken Seite. In den Frontregionen beinhaltete die chirurgische Behandlung bei zwei Tieren eine zusätzliche Zitronensäurebehandlung der zentralen Inzisiven des Unterkiefers und bei einem Tier der zentralen Inzisiven des Oberkiefers. Die zweiten Prämolaren der Unterkiefer sowie die ersten Molaren, die nicht parodontalgeweblicher Lysis ausgesetzt waren, Wurden ebenfalls chirurgisch behandelt. Nach der Mobilisierung des Lappens wurde die bukkale Knochenplatte in einem Umfang entfernt, der dem Knochenverlust entsprach, der durch das experimentelle Parodontitismodell an den Prämolaren des Oberkiefers erzeugt worden war. Die Zementschicht der chirurgisch entblössten Wurzelabschnitte wurde mittels Wurzelglättung entfernt. An allen behandelten Zähnen wurde in der Höhe des alveolaren Kieferkammes eine Kerbe in die bukkalen Wurzeloberflächen einpräpariert. Sechs Monate später wurden die Tiere geopfert und histologische Schnitte präpariert. An der Wurzeloberfläche wurden von der apikalen Grenze der einpräparierten Kerbe bis zum apikalen Abschluss des Epithelansatzes und zum alveolaren Kieferkamm, lineare Messungen vorgenommen. Die Resultate zeigen, dass eine Zementnlagerung und neue Attachmentanlage an solche Wurzeloberflächen nicht stattfand, die vorher der Einwirkung parodontaler Taschen ausgesetzt waren, danach mit Zahnsteinentfernung und Wurzelglättung behandelt wurden oder chirurgisch ihrer Knochenstütze und ihrer vorher "nicht exponierten" Zementschicht beraubt worden waren.
