Low Tube Voltage Chest Computed Tomography With Enhancement Using Low-Concentration Iodinated Contrast Media: Comparison of 240 mg/mL Versus 300 mg/mL Iodinated Contrast Media.

Purpose: To evaluate the image quality of low voltage chest computed tomography with enhancement (CECT) using low-concentration-iodine contrast media (LCCM). Method: From 9 December to 19 December 2019, three different protocols were used for 263 patients undergoing chest CECT. Chest CECT was done using routine (300 mgI/ml contrast media with 100 kVp) protocol (group 1), LCCM (240 mgI/ml contrast media)–100 kVp protocol (group 2) and LCCM–80 kVp protocol (group 3) in 91, 97 and 75 patients, respectively. The overall diagnostic acceptability, anatomical depiction, noise and contrast-related artifacts were assessed. Additionally, the mean attenuation, signal-to-noise ratio (SNR), contrast-to-noise ratio (CNR) and figure of merit (FOM) in the aorta and the main pulmonary trunk were measured. Results: The overall diagnostic acceptability scores were not significantly different between groups 1 and 2 (P =.261); group 3 demonstrated significantly lower overall diagnostic acceptability score compared with group 1 (P =.011) or group 2 (P <.001). However, in CECT with iterative reconstruction (IR), the overall diagnostic acceptability scores did not show significant difference among 3 groups. Group 3 showed significantly lower effective radiation dose compared with group 1 (2.33 vs 1.22 mSv, P <.001) or group 2 (2.28 vs.22 mSv, P <.001). Conclusions: In 100 kVp chest CECT, the image quality of using 240 mg/mL iodinated contrast media is comparable to that using 300 mg/mL iodine contrast media, regardless of application of IR; with IR, chest CECT using 80 kVp and 240 mg/mL iodinated contrast media results in acceptable image quality and lower radiation dose. Objectif: Évaluer la qualité d'une imagerie par TDM basse tension avec rehaussement (CECT) utilisant un produit de contraste à faible concentration d'iode (LCCM). Méthode: Du 9 au 19 décembre 2019, trois protocoles différents ont été utilisés chez 263 patients subissant une CECT du thorax. La CECT du thorax a été exécutée avec un protocole habituel (300 mg/mL de produit de contraste et 100 kVp; groupe 1), un protocole associant LCCM (240 mg/mL de produit de contraste) et 100 kVp (groupe 2) et un protocole associant LCCM et 80 kVp (groupe 3) chez 91, 97 et 75 patients respectivement. L'acceptabilité globale du diagnostic, la représentation anatomique, le bruit et les artefacts liés au contraste ont été évalués. De plus, l'atténuation moyenne, le rapport signal/bruit (SNR), le rapport contraste/bruit (CNR) et la figure de mérite (FM) dans l'aorte et le principal tronc artériel pulmonaire ont été calculés. Résultats: Les cotes globales d'acceptabilité du diagnostic n'affichaient pas de différences significatives entre les groupes 1 et 2 (P = 0,261); la cote globale d'acceptabilité du diagnostic était significativement inférieure pour le groupe 3, comparativement au groupe 1 (P = 0,011) et au groupe 2 (P < 0,001). Toutefois, les cotes globales d'acceptabilité du diagnostic n'étaient pas significativement différentes entre les 3 groupes avec les CECT avec reconstruction itérative. L'efficacité de la dose de radiation du groupe 3 a été significativement inférieure comparativement au groupe 1 (2,33 versus 1,22 mSv, P < 0,001) et au groupe 2 (2,28 versus 0,22 mSv, P < 0,001). Conclusions: La qualité de l'image dans la CECT de thorax à 100 kVp utilisant 240 mg/mL de produit de contraste iodé est comparable à celle utilisant 300 mg/mL, indépendamment du recours à la reconstruction itérative. Avec la reconstruction itérative, la CECT de thorax à 80 kVp utilisant 240 mg/mL de produit de contraste iodé aboutit à une qualité d'image acceptable avec une plus faible dose d'irradiation.
