不同焦虑程度青少年首发广泛性焦虑障碍患者血清 NPY、5-HT、BDNF 的变化及其与生活应激、炎症因子和记忆功能的相关性分析.

Objective: To investigate the changes of serum neuropeptide Y (NPY), 5-hydroxytryptophan (5-HT) and brain-derived neurotrophic factor (BDNF) in first-episode generalized anxiety disorder (GAD) of adolescents with different anxiety levels and their correlation with life stress, inflammatory factors and memory function. Methods: 147 adolescent first-episode GAD patients who were admitted to Chengdu mental health center from January 2019 to December 2021 were selected, and they were divided into mild anxiety group (5-9 scores, 50 cases), moderate anxiety group (10-14 scores, 65 cases) and severe anxiety group (15-21 scores, 32 cases) according to the Generalized Anxiety Scale (GAD-7). Serum levels of NPY, 5-HT, BDNF, C-reactive protein (CRP), interleukin (IL) -1αand IL-6 were detected. Students’ Life Stress Inventory (SLSI) and delayed matching test (DMS) were used to evaluate the life stress level and memory function. The levels of serum NPY, 5-HT, BDNF, CRP, IL-1α and IL-6 as well as SLSI and DMS were compared between groups, and the correlation between serum levels of serum NPY, 5-HT and BDNF and levels of serum CRP, IL-1α and IL-6 as well as SLSI and DMS were analyzed. Results: The levels of serum NPY, 5-HT and BDNF in severe anxiety group were lower than those in moderate anxiety group and mild anxiety group (P<0.05), and the moderate anxiety group was lower than the mild anxiety group (P<0.05). The levels of CRP, IL-1α and IL-6 and SLSI score in severe anxiety group were higher than those in moderate anxiety group and mild anxiety group (P<0.05). The moderate anxiety group was higher than the mild anxiety group (P<0.05). The total delayed response time and no delayed response time in the severe anxiety group were longer than those in the moderate anxiety group and the mild anxiety group (P<0.05), and the moderate anxiety group was longer than the mild anxiety group (P<0.05). The total delayed correct number and no delayed correct number in severe anxiety group were less than those in moderate anxiety group and mild anxiety group (P<0.05),and the moderate anxiety group was less than the mild anxiety group (P<0.05). The levels of serum NPY, 5-HT and BDNF were negatively correlated with SLSI score, CRP, IL-1α, IL-6, total delayed response time and no delayed response time (P<0.05), and positively correlated with total delayed correct number and no delayed correct number (P<0.05). Conclusion: With the aggravation of anxiety, life stress intensity, inflammatory factor level and memory function of adolescent first-episode GAD patients are enhanced, and all of them are related to the decrease of the levels of serum NPY, 5-HT and BDNF. 目的: 探討不同焦慮程度青少年首發廣泛性焦慮障礙 (GAD) 患者血清神經肽Y (NPY)、5-羥色胺 (5-HT)、腦源性神經營養因子 (BDNF) 的變化及其與生活應激、炎癥因子和記憶功能的相關性. 方法: 選擇2019年1月至2021年12月成都市精神衛生中心收治的147例青少年首發GAD患者, 根據廣泛性焦慮量表 (GAD-7) 分為輕度焦慮組 (5-9分, 50例)、中度焦慮組 (10-14分, 65例) 、重度焦慮組 (15-21分, 32例). 檢測血清NPY、5-HT、BDNF、C反應蛋白 (CRP)、白細胞介素 (IL) -1α、IL-6水平, 采用學生生活應激問卷 (SLSI)、延遲匹配測驗 (DMS) 評估生活應激水平和記憶功能. 比較組間血清NPY、5-HT、BDNF、CRP、IL-1α、IL-6水平以及SLSI、DMS差異, 分析血清NPY、5-HT、BDNF水平與血清CRP、IL-1α、IL-6水平及SLSI、DMS的相關性. 結果: 重度焦慮組血清NPY、5-HT、BDNF水平低于中度焦慮組和輕度焦慮組 (P<0.05), 且中度焦慮組低于輕度焦慮組 (P<0.05), 重度焦慮組CRP、IL-1α、IL-6水平以及SLSI評分高于中度焦慮組和輕度焦慮組 (P<0.05), 且中度焦慮組高于輕度焦慮組 (P<0.05). 重度焦慮組總延遲反應時間、無延遲反應時間長于中度焦慮組和輕度焦慮組 (P<0.05), 且中度焦慮組長于輕度焦慮組 (P<0.05); 重度焦慮組總延遲正確數、無延遲正確數少于中度焦慮組和輕度焦慮組 (P<0.05), 且中度焦慮組少于輕度焦慮組 (P<0.05). 青少年首發GAD患者血清NPY、5-HT、BDNF水平與SLSI評分、CRP、IL-1α、IL-6、總延遲反應時間、無延遲反應時間呈負相關 (P<0.05), 與總延遲正確數、無延遲正確數呈正相關 (P<0.05). 結論: 青少年首發GAD患者隨著焦慮程度加重, 其生活應激強度增強、炎癥因子水平升高, 記憶功能減弱, 且均與患者血清NPY、5-HT、BDNF水平降低有關.
