DENSITATEA MINERALĂ OSOASĂ ŞI COMPOZIŢTIA CORPULUI LA BOLNAVI CU SPONDILITĂ ANCHILOZANTĂ PRECOCE.

Osteoporosis (OP) is a well-recognized feature of ankylosing spondylitis (AS), but the time of onset, predilect sites and pathogenesis remains controversial. Also the influence of disease on total body and soft tissue composition was only recently studied and there is little data in the current literature. Objectives: To determine total body and regional bone mineral density (BMD) and to examine the changes in body composition in a cohort of patients with early AS. Material and methods: Twenty-one patients (17 males, 4 pre-menopausal females) with AS without syndesmophytes and coxofemural joint destruction, with mean age 34 years, were compared with 21 sex- and age-matched controls. Lumbar spine and total hip BMD were evaluated by dual energy X-ray absorbtiometry with a Lunar Prodigy Advance device (GE). Total body measurements were done with the same device, and BMD of the whole body and body composition were studied. Results: In patients with early AS, BMD was reduced in both lumbar spine 1.11 ± 0.17 g/cm² (T score -0.95 ± 1.52) and total hip 0.97 ±0.12 g/cm² (T score -0.63 ± 0.93) as compared with controls (1.24 ± 0.14 g/cm² [T score 0.26 ± 1.2], respectively 1.07 ± 0.0 8 g/ cm² [T score 0.10 ± 0.85]; all p < 0.05). Total body BMD was also significantly lower in AS patients (p < 0.05). We found that total body BMD was correlated with spine BMD (r = 0.70, p = 0.006), but not with total hip BMD. On the contrary, bone mineral content (BMC), fat and lean masses did not differ between patients and controls (all p > 0.05). According to the WHO definition of osteoporosis, we found spine osteopenia or osteoporosis in 52.3% patients, while 38% had total hip osteopenia (none had total hip osteoporosis). No significant correlation was found between BMD at any site, variables of body composition and disease duration or biological markers of disease activity (erythrocyte sedimentation rate and C-reactive protein serum levels). Conclusions: AS is associated with generalized bone loss that occurs early in the disease course. On the other hand, AS has no influence on the variables of body composition (BMC, fat and lean masses), at least not in the early stages of the disease. [ABSTRACT FROM AUTHOR]
Osteoporoza (OP) este o manifestare extra-articulară bine cunoscută a spondilitei anchilozante (SA), dar momentul debutului, distribuţia şi patogeneza acesteia sunt controversate. De asemenea, influenţa bolii asupra compoziţiei corpului a fost doar recent studiată şi există puţine date în literatură. Obiective: Evaluarea densităţii minerale osoase (DMO) la nivelul întregului schelet şi în diferite regiuni şi examinarea modificărilor compoziţiei corpului într-un grup de pacienţi cu SA precoce. Material şi metodă: 21 de pacienţi (17 bărbaăi şi 4 femei) cu SA, care nu aveau sindesmofite şi afectarea şoldului, cu vârsta medie de 34 ani au fost comparaţi cu 21 de subiecţi sănătoţi. Evaluarea DMO s-a făcut la nivelul coloanei vertebrale lombare şi şoldului total prin metoda absorbţiometriei cu fascicul dual de raze X (DEXA) cu un aparat Lunar Prodigy Advance (GE). Cu acelaşi aparat au fost efectuate măsurători total body şi s-au studiat DMO la nivelul întregului schelet şi compoziţia corpului. Rezultate: La pacienţii cu SA precoce, DMO a fost semnificativ redusă atât la nivelul coloanei vertebrale lombare 1,11 ± 0,17 g/cm² (scorul T -0.95 ± 1.52), cât şi la nivelul şoldului total 0,97 ± 0,12 g/cm² (scorul T -0.63 ± 0.93), în comparaăie cu martorii (1,24 ± 0,14 g/cm² [ scorul T 0.26 ± 1.2], respectiv 1,07 ± 0,08 g/cm² [scorul T 0.10 ± 0.85]; p < 0.05). De asemenea, DMO la nivelul întregului schelet evaluată prin total body a fost semnificativ scăzut&10#x; la pacienţii cu SA (p < 0.05). Valorile DMO măsurate prin total body s-au corelat cu valorile DMO spinale (r = 0,70, p = 0,006), dar nu şi cu cele de la nivelul oldului total. În contrast, valorile conţinutului mineral osos (CMO), masei slabe şi ᘳ;esutului adipos ale pacienţilor spondilitici au fost similare cu ale martorilor (p > 0.05). Conform definiţiei OMS a OP, am găsit osteopenie sau OP lombară la 52,3% dintre pacienţi, ín timp ce 38% dintre pacienţi au avut osteopenie la nivelul şoldului total (nici un pacient nu a avut OP la acest sediu). DMO la nici un sediu şi nici una dintre variabilele compoziţiei corpului nu s-au corelat cu durata bolii sau parametrii biologici de activitate a bolii (viteza de sedimentare a hematiilor şi proteina C-reactivă). Concluzii: SA se asociază cu pierdere de masă osoasă, care este generalizată ăi apare precoce în evoluţia bolii. în contrast, SA nu pare să influenţeze variabilele principale ale compoziţiei corpului (CMO, masa slabă şi ţesutul adipos), cel puţin nu în stadiile precoce de boală.
