Background: The target of quality management is to improve medical care through a review process of concrete benchmarks. By order of the highest medical authority of the Tyrolean emergency services, the internal indicator analysis (IA) of prehospital treatment in 2013 was developed, followed by the creation of the first Tyrolean benchmark analysis (BA) for the first half of 2015. The aims were to check the validity, determine discrepancies, re-evaluate some of the BA parameters in greater depth, and communicate the BA to the EMS bases. Methods: Tactical deployment intervals and medical benchmarks (MB) relevant for EMS were compared between the IA and BA to identify relevant discrepancies. An error ratio of 10% has been taken into account when the MB target values were defined. Results: A 5% discrepancy between the IA and BA for 'symptom onset' and 'blood glucose monitoring' in the indicator 'stroke' was found. When the treatment period at the emergency location was longer for the indicator 'ACS' comparing EMS bases, then it was also longer for the indicator 'stroke'. Conclusion: The treatment time is longer for ACS than for stroke because more medical monitoring tasks are needed for ACS. It is unclear why the EMS-base order for the 50 percentile does not correlate with the 90 percentile. [ABSTRACT FROM AUTHOR]
Hintergrund: Qualitätsmanagement zielt auf die Erhöhung der Versorgungsqualität durch die Überprüfung konkreter Qualitätsmaßstäbe ab. Im Auftrag des Ärztlichen Leiters Rettungsdienst (ÄLRD) Tirol wurde ein interner 'Kennzahlenbericht zum prähospitalen Notfallprozess 2013 (KZB)' und in Folge der Erste Tiroler NEF-Benchmarkbericht 1. Hj 2015 (BB) erstellt. Ziel war, die Validität zu überprüfen, Abweichungen zu erkennen, einzelne Parameter tiefgründiger zu untersuchen sowie den BB unter den Notarztstützpunkten zu kommunizieren. Methode: NEF-relevante einsatztaktische Intervalle und medizinische Qualitätsindikatoren (QI) zwischen KZB und BB wurden ausgewertet und verglichen. Die QI wurden gegenübergestellt und auf relevante Abweichungen überprüft. Unter Annahme einer 10 % Dokumentationsschwäche sowie weiterer Faktoren wurden Zielerreichungsgrade für die QI vorgegeben. Ergebnisse: Zwischen KZB und BB bei dokumentiertem Symptom-Onset und bei 'Blutzucker gemessen' bei Schlaganfall (Stroke) ergab sich eine 5 % Abweichung. Wenn die Versorgungszeit am Notfallort bei ACS (akutes Koronarsyndrom) länger war, dann war das auch bei Schlaganfall so. Schlussfolgerung: ACS-Versorgungszeiten sind länger als bei Schlaganfall, da mehr ärztliche Aufgaben anfallen. Unklar ist, warum die Reihung der Stützpunkte nach Versorgungszeit im 50er- nicht mit der im 90er-Perzentil korreliert. [ABSTRACT FROM AUTHOR]
