SANAL GERÇEKLİK GÖZLÜĞÜ ve AKILLI TELEFON OYUN UYGULAMASININ 7-13 YAŞ GRUBU ÇOCUKLARDA PREOPERATİF ANKSİYETE ÜZERİNE ETKİSİ. (Turkish)

The present study consists of two phases. The first phase was conducted methodologically with the aim of Turkish validity and reliability study of Children's Perioperative Multidimensional Anxiety Scale (CPMAS). In these cond phase, this randomized controlled experimental study was conducted to determine the effect of virtual reality glass and smart phone game application on preoperative anxiety in 7-13 years children. The research was carried out between the ages of 7-13 years children under going day surgery and August 2018- March 2020 otolaryngology and pediatric surgery clinics of a university hospital. Ethics committee, institutional consent and parental consent were obtained. In the first phase, the CPMAS was applied to children under going day surgery (COO = preoperative assessment on 100 children, COG= on the day of the operation on 73children, COS= one month postoperatively on 43 children). In the second phase, age, sex, surgery type, previous hospitalization experience and having chronic disease criteria of the children in control group (n=20), smart phone game application (n=20) and virtual reality glass game application (n=20) groups. Data were collected with Descriptive Characteristics Form for Children and Families, CPMAS and The State- Trait Anxiety Inventory for Children. Before the surgical operation, the game with a smart phone and virtual reality glass was played to the intervention/game groups for 15 minutes. Physiological parameters of children were measured before, after and 90 minutes after the operation. In addition, CPMAS was applied to children in COO, COG and COS. Data were analyzed with factor analysis, correlation analysis, post-power analysis, descriptive statistics, one way analysis of Kruskall-Wallis and Friedman Tests for the repeated measurements. In the Turkish validity and reliability phase of the study, it was determined that CPMAS is a valid and reliable scale that can be used to evaluate the perioperative anxiety of Turkish children (Cronbachalpha = 0.78-0.81). In these cond phase of the research; preoperative anxiety and heart rate values of children playing game with virtual reality glass after intervention were low (p <0.05). According to these results; it may be recommended that health care professionals working with children under going surgical operations should use CPMAS to diagnose preoperative anxiety in children and use distraction techniques such as virtual reality glass to reduce preoperative anxiety. Bu araştırma iki aşamadan oluşmaktadır. Birinci aşama; Çocuklarda Çok Boyutlu Perioperatif Anksiyete O lçeğ i'nin (ÇÇBPAO ) Tu rkçe ğeçerlik ğu venirlik çalışması amacı ile metadolojik olarak yapılmıştır. İ kinci aşama ise; sanal ğerçeklik ğo zlu ğ u ve akıllı telefon oyun uyğulamasının 7-13 yaş ğrubu çocuklarda preoperatif anksiyete u zerine etkisini değ erlendirmek amacıyla randomize kontrollu deneysel çalışma olarak yapılmıştır. Araştırma, bir u niversite hastanesinin Kulak Burun Boğ az ve Çocuk Cerrahi bo lu mlerinde ğu nu birlik cerrahi operasyon uyğulanan çocuklar ile Ağ ustos 2018-Mart 2020 tarihinde yapılmıştır. Bu çalışmada etik kurul onayı, kurum izni, çocuk ve ebeveyn onamı alınmıştır. Araştırmanın birinci aşamasında, ğu nu birlik cerrahi operasyon uyğulanan çocuklara (COO = cerrahi operasyonun planlandığ ı ğu n 100 çocuğ a, COG= cerrahi operasyon ğu nu 73 çocuğ a, COS= cerrahi operasondan 1 ay sonra 43 çocuğ a) ÇÇBPAO uyğulanmıştır. Araştırmanın ikinci aşamasında; akıllı telefon (n=20), sanal ğerçeklik ğo zlu ğ u (n=20) oyun uyğulaması ve kontrol ğrubundaki (n=20) çocukların yaş, cinsiyet, cerrahi operasyon tu ru, daha o nce hastaneye yatma deneyimi ve kronik hastalığ a sahip olma kriterleri benzer seçilmiştir (p>0.05). Veriler, Çocuklar ve Aileleri için Tanıtıcı O zellikler Formu, ÇÇBPAO ve Çocuklar için Durumluk Kayğı Envanteri ile toplanmıştır. Cerrahi operasyondan o nce, ğirişim/ oyun ğruplarına 15 dakika akıllı telefon ve sanal ğerçeklik ğo zlu ğ u ile oyun oynatılmıştır. Oyun uyğulaması o ncesi, sonrası ve postoperatif 90. dakikada çocukların fizyolojik parametreleri o lçu lmu ştu r. Ayrıca çocuklara COO, COG ve COS'de ÇÇBPAO uyğulanmıştır. Verilerin değ erlendirilmesinde tanımlayıcı istatistikler, fakto r analizi, korelasyon analizi, post-power analizi, Kruskal Wallis Varyans Analizi, tekrarlı o lçu mlerde Friedman Testi kullanılmıştır. Araştırmanın Tu rkçe ğeçerlik ve ğu venirlik aşamasında ÇÇBPAO 'nin Tu rk çocuklarının perioperatif anksiyetelerini değ erlendirmede kullanılabilir, ğeçerli ve ğu venilir bir o lçek olduğ u saptanmıştır (Cronbach alfa= 0.78-0.81). Araştırmanın ikinci aşamasında; oyun uyğulaması sonrası sanal ğerçeklik ğo zlu ğ u oyun ğrubundaki çocukların preoperatif anksiyetelerinin ve kalp atım hızı değ erlerinin daha az olduğ u bulunmuştur (p<0.05). Bu sonuçlara ğo re; cerrahi operasyon uyğulanan çocuklarla çalışan sağ lık profesyonellerinin çocuklarda preoperatif anksiyeteyi tanılamak için ÇÇBPAO 'ni kullanmaları ve preoperatif anksiyeteyi azaltmak için sanal ğerçeklik ğo zlu ğ u ğibi dikkat dağ ıtma tekniklerini kullanmaları o nerilebilir.
