Fris Verder: een verkennend onderzoek naar groepsbehandelingen voor ouderen met alcoholproblemen. (Dutch/Flemish)

Introduction: Although excessive alcohol use among the elderly is prevalent, available group-based treatment programs are not standardized. This paper describes a study on four group-based programs for elderly with alcohol problems, based on the Dutch program 'Fresh Onward'. The aim was to explore these programs, present characteristics and similarities/differences between the programs, explore possibilities for standardization, and compare current evidence-based elements from the literature with the four selected programs in this study. Methods: A document analysis, interviews with implementers and practitioners, and evidence-based insights from the literature were generated. Results: Prominent similarities between the programs showed that all strived for acceptable use of alcohol such as abstinence, all were group-oriented and based on lifestyle training/cognitive behavioral therapy (CBT). All programs were provided within specialized mental health care, mainly by nurses. Programs all ranged between 8–12 participants per group. Participants were referred mainly through primary care. Also, all programs paid attention to psychoeducation and life stage problems. Prominent differences between the programs were the group structure they used (open versus closed), the duration, and the theoretical premise of the programs. Additional to lifestyle training/CBT, the acceptance and commitment therapy (ACT) was also conducted. Elements from the literature that match the programs, were not patronizing, providing attention to increasing individual skills, information transfer, influencing social norms and providing tailored feedback based on monitoring of drinking behavior. Conclusion: The current programs all contained effective and evidence-based elements. In future research, an evaluation should be conducted to assess the effectiveness in advance of standardization on a national level. Also, the expansion and subsequent repositioning of 'Fresh Onward' programs to primary care facilities in the Dutch healthcare system could increase accessibility and intake of participants, and should not be ignored in future research. Samenvatting: Inleiding: Hoewel overmatig alcoholgebruik onder ouderen voorkomt, zijn de groepsbehandelprogramma's niet gestandaardiseerd. In dit onderzoek staan vier programma's voor ouderen met alcoholproblematiek centraal, met als koepelnaam Fris Verder. Het doel is een verkenning uit te voeren naar kenmerken en overeenkomsten/verschillen, mogelijkheden voor standaardisatie en evidente/werkzame elementen uit de literatuur, die we vergelijken met de kenmerken van de programma's. Methode: We voerden een documentanalyse, interviews met professionals en een vergelijking met literatuur uit. Resultaten: Prominente overeenkomsten waren de volgende: de doelen die deelnemers en therapeuten/trainers nastreven (aanvaardbaar gebruik, waaronder abstinentie), de groepsgerichtheid van de programma's, de programma's waren gebaseerd op leefstijltraining/cognitieve gedragstherapie (CGT), ze werden uitgevoerd binnen de gespecialiseerde ggz door vooral verpleegkundigen, een spreiding van acht tot twaalf deelnemers, verwijzingen verliepen vooral via de eerstelijnszorg, en er was aandacht voor psycho-educatie en levensfaseproblematiek. Er waren ook verschillen: in groepsstructuur (open/gesloten), duur en theoretische uitgangspunten (leefstijltraining, CGT en/of acceptatie en toewijdingstherapie (ACT)). De volgende werkzame elementen uit de literatuur sluiten aan: geen betutteling, aandacht voor het vergroten van de individuele vaardigheden en informatieoverdracht, het beïnvloeden van sociale normen en het leveren van gepersonaliseerde feedback. Conclusie: De huidige programma's bevatten alle effectieve elementen. Voor standaardisering wordt doorgevoerd en landelijk effectonderzoek wordt gedaan, is een evaluatie nodig om de effectiviteit van de programma's te bepalen met alcoholgebruik als uitkomstmaat, en met aandacht voor een inventarisatie van Fris Verder in een eerdere fase in de zorgketen.
