Conduite automobile : Quelle est la pertinence et l'utilité de l'échelle de Reisberg en 2023 ? (French)

The publication of the decree on the care of people with neurocognitive disorders brought to the fore the Reisberg's Global Deterioration Scale, a scale that only few clinicians use in memory centers or in geriatric. This scale has a number of limitations, not least of which is that it is obsolete, since it does not take into account disease advances in scientific knowledge with biomarkers. Consequently, the stages evoked no longer correspond to current descriptions. Moreover, it only concerns Alzheimer's disease, whereas in our practice we encounter other neurodegenerative pathologies. Even if we decide to use another global assessment scale, such as the Clinical Dementia Rating or the Functional Assessment Staging, they cannot replace a personalized assessment. Indeed, it is important to stress that this decree does not take into account the relevance of personalized assessments using, for example, neuropsychological tests to estimate driving ability. A personalized assessment accompanied by a real-life driving test would be preferable than a score on a global scale. This article therefore presents the Global Deterioration Scale, highlighting its unsuitability for assessing whether or not to continue driving. [ABSTRACT FROM AUTHOR]
Résumé: La publication de l'arrêté sur la conduite automobile pour des personnes -présentant des troubles neurocognitifs a mis en avant l'échelle d'évaluation globale, la Global Deterioration Scale de Reisberg, que peu de cliniciens utilisent en France dans les consultations mémoire (neurologique ou gériatrique). Cette échelle présente plusieurs limites ; elle ne prend notamment pas en considération l'avancée des connaissances scientifiques et ne concerne que la maladie d'Alzheimer alors que dans nos exercices, nous rencontrons d'autres pathologies neuro-évolutives. Même si nous décidions d'utiliser une autre échelle d'évaluation globale comme la Clinical Dementia Rating ou la Functional Assessment -Staging, elles ne peuvent pas remplacer une évaluation personnalisée et multidisciplinaire. En effet, il est important de souligner que cet arrêté ne prend pas en considération la pertinence des évaluations personnalisées à l'aide, entre autres, de tests neuropsychologiques permettant d'estimer l'(in)aptitude à la conduite automobile. Cet article propose donc de réaliser une présentation de la Global Deterioration Scale afin de mettre en évidence son inadéquation dans l'appréciation de la poursuite ou non de la conduite automobile.
