Comparison of regional pulmonary perfusion in lobar pneumonia during high frequency and conventional mechanical ventilation in sheep.

We compared the effects of high frequency jet ventilation (HFV), conventional ventilation (CMV), and spontaneous breathing (SB) on regional pulmonary blood flows (Qlll), standard cardiopulmonary measurements and the serum levels of the first generation metabolites of prostacyclin (6-keto-PGFα) and thromboxane A (TxB) in established left tower lobe pseudomonas aeroginosa pneumonia in 11 sheep. Gram negative pneumonia resulted in significant increases in alveolar-arterial oxygen gradients [(A-a)DO] and pulmonary shunt fractions (QslQt), as well as a significant decrease in Qlll during SB. Significant differences in standard haemodynamics, (A-a)DO, Qs/Qt, and Qlll were not observed when HFV was compared to CMV. However, serum levels of 6-keto-PGFα were elevated when the animals underwent HFV. We conclude that HFV is a safe and efficient method of oxygenation and ventilation in unilobar gram negative pneumonia and also results in a significant increase in the serum levels of 6-keto-PGFα when compared to CMV in sheep. On a comparé les effets de la ventilation à haute fréquence enjet (HFV), la ventilation conventionnelle (CMV), et la respiration spontanée (SB) sur les flots régionaux pulmonaires (Qlll), les paramétres hémodynamiques standards et le niveau sérique des métabolites de premiére génération de la prostacycline (6-keto-PGFα et le thromboxane A (TxB) chez 11 moutons atteints ďune pneumonie lobaire inférieure gauche à pseudomonas aéroginose. La pneumonie à gramme négatif a amené ľaugmentation significative des gradients alvélo-artériels de ľoxygéne [(A-a)DO] et des fractions de shunt pulmonaire (Qs/Qt), ainsi qu'une diminution significative du Qlll durant la respiration spontanée. Des différences significatives dans des paramétres hémodynamiques, (A-a)DO, Qs/Qt, et Qlll n'étaient pas observées quand le HFV était comparé au CMV. Cependant, le niveau sérique du 6-keto-PGFα était élevé quand les animaux étaient sous HFV. On conclue que le HFV est sécuritaire et efficace pour ľoxygénation et la ventilation lors ďune pneumonie unilobaire à gramme négatif et qu 'elle améne une augmentation significative des niveaux sériques du 6-keto-PGFα: comparativement à la CMV chez les moutons.
