Introduction: Functional magnetic resonance urography (fMRU) is a (semi-)invasive examination, which fuses a superb anatomicmorphologic and quantitative functional imaging of the urinary system in one method. Purpose: In this paper we will focus on the MR urinary system imaging technique with the emphasis on fMRU. We will discuss its advantages and disadvantages as well as its indications in childhood. Methods: There are several MRU techniques. The selection depends on the clinical issue, renal function and clinical condition of the child. MRUs are divided into two groups: an anatomical and morphological in which morphology is displayed and which includes the static-liquid MRU (T2 weighted sequences: T2 HASTE, T2 SPACE, T2 TSE) and the conventional MRU (T1 VIBE) in which the excretion of i.v. administered paramagnetic CM in the excretory system is displayed and a detailed morphological view, usually by using a diuretic (furosemide). The second group represents a functional MRU (fMRU) in which the use of a gradient echo sequence T1 Turbo Flash enables a continuous imaging passage of CM through the renal parenchyma in the excretory system of the kidney and ureter. The images are analysed in a special program which allows an analysis of renal function comparable to the scintigraphy analysis. The measurements are made and adjusted to MR Siemens Aera 1.5 T. Results and discussion: In the recent years, MRU/fMRU has become an important diagnostic tool, complementary to the ultrasound examination in detection and classification of congenital abnormalities of the urinary system, determining the dilatation of a cavern system and the centre of obstruction in the urinary system. It represents the second step in the diagnostic imaging, immediately following the ultrasound examination of the urinary system. Conclusion: MRU is the only method in which we can combine the morphology and function of the urinary system (an "all-inone investigation"), without the use of ionizing radiation. The diagnostic value of MRU/fMRU has been proven and accepted in the international protocols of treatment of various diseases of the urinary system. Uvod: Funkcijska magnetno resonančna urografija (fMRU) je (semi)invazivna preiskava, ki hkrati omogoča anatomski in morfološki prikaz sečil ter opredeli funkcijo ledvic v eni preiskavi. Namen: V prispevku se bomo posvetili tehniki slikanja sečil z magnetno resonanco, s poudarkom na fMRU, njenim prednostim in slabostim ter indikacijam v otroškem obdobju. Metode dela: Poznamo več MRU tehnik. Za katero se bomo odločili, je odvisno od kliničnega vprašanja, ledvične funkcije in kliničnega stanja otroka. MRU delimo v dve skupini; anatomsko-morfološko, pri kateri je v ospredju prikaz morfologije in vključuje statično-tekočinsko MRU (T2 poudarjena pulzna zaporedja; T2 HASTE, T2 SPACE, T2 TSE) in klasično MRU (Tl VIBE), pri kateri je poudarjen prikaz izločanja i.v. apliciranega paramagnetnega KS v votli sistem in natančen morfološki prikaz le-tega, običajno ob pomoči diuretika (furosemid). Druga skupina predstavlja funkcijsko MRU (fMRU), pri kateri z uporabo gradientnega zaporedja Tl Turbo Flash omogočamo kontinuiran zapis prehoda KS skozi ledvični parenhim v votli sistem ledvic in pripadajoči sečevod. Ta zapis vnesemo v poseben program, ki omogoča analizo funkcije ledvic, primerljivo s scintigrafsko analizo. Meritve so narejene in prilagojene MR tomografu Siemens Aera 1.5 T. Rezultati in razprava: MRU/fMRU je v zadnjih letih postala pomembno diagnostično orodje, komplementarna ultrazvočni preiskavi v odkrivanju in klasifikaciji prirojenih nepravilnosti sečil, pri opredelitvi dilatacije votlega sistema in pri določanju mesta obstrukcije v področju urotrakta. Predstavlja drugo stopnjo v slikovni diagnostiki, takoj za UZ preiskavo sečil. Zaključek: MRU je edina metoda, pri kateri lahko združimo morfologijo s funkcijo sečil (»vse v eni preiskavi«) brez uporabe ionizirajočega sevanja. Diagnostična vrednost MRU/fMRU je dokazana in sprejeta v mednarodne protokole za obravnavanje različnih obolenj sečil. Na Pediatrični kliniki v Ljubljani izvajamo vse vrste MR tehnik v vseh starostnih obdobjih otrok. [ABSTRACT FROM AUTHOR]
