Antipsychotics and antiemetics are commonly prescribed medications across medical specialties. Antipsychotics are used to manage conditions such as psychosis, schizophrenia, bipolar disorder, depression, acute delirium, acute agitation, generalized anxiety disorder and Tourette syndrome. Antiemetics are used to manage nausea and vomiting, mainly in the setting of pregnancy and chemotherapy, motion sickness, gastroparesis and migraines. One of the fatal adverse effects of these medications is neuroleptic malignant syndrome (NMS), which carries high morbidity and mortality. This article identifies the common agents implicated in NMS, the pathophysiology of the syndrome, the clinical signs and symptoms, the risk factors, the laboratory and imaging findings, and treatment strategies.
