Effects of estradiol fluctuations during menstrual cycle on motor abilities in physically active woman

Značajna kolebanja u razinama ženskih spolnih hormona tijekom menstruacijskog ciklusa (MC) žene odražavaju se na rad i funkcioniranje ne samo reproduktivnih organa, nego i brojnih drugih organskih sustava. Pronalazak estrogenskih receptora u mišićnim stanicama skeletnih mišića te fibroblastima tetiva i ligamenta, daje naslutiti da estradiol (E2) ima određeni utjecaj na mišićni sustav. Međutim, učinci E2 na mišićni sustav kao i promjene u motoričkim sposobnostima u odnosu na kolebanja razine E2 tijekom MC nisu u potpunosti istraženi, a postojeći nalazi su konfliktni. Cilj ove disertacije bio je utvrditi događaju li se promjene u motoričkim sposobnostima akutno (trenutno) ili pak vremenski odgođeno u odnosu na kolebanja razine E2 tijekom MC. U skladu s dosadašnjim spoznajama postavljene su sljedeće opće hipoteze: H1 - ne postoje statistički značajne akutne promjene u promatranim motoričkim sposobnostima u odnosu na kolebanja razine estradiola tijekom različitih faza menstruacijskog ciklusa u mladih, tjelesno aktivnih žena, i H2 - postoje statistički značajne vremenski odgođene promjene u promatranim motoričkim sposobnostima tijekom menstruacijskog ciklusa u odnosu na kolebanja razine estradiola. Hipoteze su testirane eksperimentom u kojem su ispitanice mjerene tijekom 5 različitih faza MC (rana folikularna faza, periovulacija, rana luteinska faza i kasna luteinska faza). 1) U ranoj folikularnoj fazi, periovulaciji i sredini luteiske faze mjerenja su provedena zbog testiranja potencijalnih akutnih promjena u motoričkim sposobnostima u odnosu na kolebanja razine E2 tijekom MC, 2) a u ranoj i kasnoj luteinskoj fazi za testiranje postojanja vremenski odgođenih promjena u motoričkim sposobnostima u odnosu na kolebanja razina E2 tijekom MC. Uzorak ispitanica su činile mlade, zdrave, tjelesno aktivne žene u kojih su hormonskom dokumentacijom potvrđeni normalni, ovulacijski ciklusi. Mjerenja su se sastojala od tri skupine testiranja: biokemijska analiza sljedećih parametara (E2, progesteron (P4), testosteron (T), luteinizirajući hormon (LH), folikulostimulirajući hormon (FSH), dehidroepiandrostedion-sulfat (DHEAS), globulin koji veže spolne hormone (SHBG) i indeks slobodnih androgena (FAI)); antropometrijska mjerenja i procjena motoričkih sposobnosti motoričkim testovima (vertikalni skok s pripremom bez zamaha ruku (CMJ), maksimalni poskoci u mjestu (CJs), izometrijska jakost stiska šake (JSŠ), iz ležanja na leđima maksimalno unilateralno odnoženje (ODN) i trčanje na 20 yardi (20 Y). Rezultati ovog istraživanja pokazuju značajno opadanje indeksa reaktivnosti u razdoblju predovulacijskog LH skoka, pokazuju da postoje kratke i prolazne promjene u reaktivnoj snazi koje se događaju u razdoblju predovulcijskog LH skoka odnosno vršnih vrijednosti E2 tijekom ovulacijskog ciklusa, međutim ne i u ostalim motoričkim sposobnostima. Unatoč očekivanjima dobiveni rezultati upućuju da nema vremenski odgođenih promjena u motoričkim sposobnostima, u odnosu na kolebanja razine E2 tijekom normalnih, ovulacijskih ciklusa. Dobiveni nalazi produbljuju naše znanje o specifičnostima mišićne funkcije žena te bi mogli imati značajne implikacije u prostoru rehabilitacije i treninga. Significant fluctuations in the plasma levels of female sex hormones during mestrual cycle (MC) reflect the work and functioning of not only the reproductive organs, but also many other tissues, organs and systems. Findings of estrogen receptors in the skelatal muscle cells and fibroblast cells, makes skeletal muscle, tendon and ligament tissue potential estrogen target tissues. However, effects of estradiol (E2) on skeletal muscle fluctuation, and changes in motor abilities compared to E2 fluctuations over the MC has not been fully explored, and the existing results are conflicting. The main goal of this study was to determine are the changes in different motor abilities acute or time-delayed in regard to E2 fluctuations over the normal, ovulatory MC. According to results of previous research two hypotheses were set up: H1 - there are no significant acute changes in the observed motor abilities in regard to E2 fluctuations during the different phases of the menstrual cycle in young, physically active women, and H2 - there are significant time-delayed changes in the observed motor abilities over the MC, in regard to E2 fluctuations. Hypotheses were tested in an experiment where subjects were measured during five different phases of MC (early follicular phase, periovulation, early luteal phase and late luteal phase. 1) To test the acute changes in motor abilities in regard to E2 fluctuations during MC, measurements were carried out in the early folicular phase, periovulatory phase and in the middle luteal phase, 2) to test the existence of time-delayed changes in different motor abilities in regard to E2 fluctuation over the MC, measurements were carried out in the early luteal and in the late luteal phase. Young, healthy and physically active women who had confirmed normal ovulatory cycles participated in this study. Measurements consisted of three groups of tests: 1) biochemistry analysis of E2, Progesterone (P4), Testosterone (T), Luteinizing hormone (LH), Follicle-stimulating hormone (FSH), Dehidroepiandrostedion sulphate (DHEA), Sex HormoneBinding Globulin (SHBG) and the Free androgen index (FAI); 2) anthropometric measurements; and 3) motor tests (counter movement jump (CMJ), vertical hopping (CJs), maximal isometric hand grip strength (JSS), unilateral hip abduction (ODN) and 20 yards (20 Y) . It was observed significant,short term, transient decline in the index of reactivity in the period of periovulation LH peak during ovulatory cycles. There were no significant changes in the other measured motor abilities during different phases of MC. So, the results of this experiment showed that there is a change in the results of reactive power at the period of highest E2 values during the ovulatory cycles, but not in the other motor abilities. Despite expectations results indicate that there is no time-delayed changes in motor abilities compared to the fluctuations E2 levels over normal, ovulatory cycles. Results of this experiment extend our knowledge about the specifics of women's muscle function, and they could have significant implications in the area of rehabilitation and training.
