L’ús de la mel en la cicatrització de les ferides: Revisió sistematitzada

Introducció: les ferides cròniques són aquelles que es caracteritzen per una evolució prolongada al llarg del temps com a resultat de l’estancament en alguna etapa del procés de curació, de manera que no es recupera la integritat anatòmica o funcional del teixit lesional. Gràcies als avenços de la medicina moderna, l’esperança de vida de la població ha augmentat, cosa que ha provocat un envelliment de la població i, proporcionalment, ha augmentat la cronicitat de les ferides a causa de la fragilitat de la pell, la debilitat del sistema immunitari i endocrí típic població gran. A més, l'abús d'antimicrobians ha augmentat l'aparició de bacteris resistents que afavoreixen encara més l'estancament en el procés de curació a causa de l'aparició de teixit no viable. En relació amb aquest estancament de la cicatrització de les ferides, s'ha conclòs que es necessita més investigació i que els tractaments alternatius com la mel no tenen prou investigació. Objectius: aquesta revisió sistematitzada té per objectiu conèixer l’ús de la mel en la cicatrització de les ferides, especialment per aquelles més cròniques, com ho són les lesions per pressió, les lesions per estasis, les isquèmiques i el peu diabètic. Metodologia: per elaborar la revisió sistematitzada s’han creat diferents estratègies de cerca per no perdre informació referent a l’aplicació tòpica de la mel en les ferides. Totes les estratègies de cerca s’han aplicat a les bases de dades següents: Pubmed, CINHAL, Scopus, Cuiden Plus, COCHRANE, LILACS i Scielo. El procés de cerca s’ha realitzat des del Desembre 2020 fins al Gener 2021 inclòs Conclusions: malgrat l’interès creixent d’utilitzar la mel com alternativa en la curació de les ferides, l’ús en la pràctica clínica és poc rellevant. La mel destaca per les seves propietats, composta majoritàriament per sucre, que li dona una textura viscosa i una alta osmolaritat, que, juntament amb el seu pH acidificat, li dona propietats antimicrobianes. La mel és més eficaç que la cura salina convencional perquè augmenta el rati de curació i disminueix el temps de cicatrització. També sembla que disminueixi el dolor i olor. Així mateix, es considera una bona alternativa per intentar prevenir l’amputació del peu diabètic. En les lesions per pressió també s’obté una millora si s’aplica mel, en canvi, en les lesions d’estasis l’aplicació de mel juntament amb compressió no presenta canvis destacables. No obstant això, caldria seguir investigant i realitzar estudis amb més participants i amb l’aplicació de doble cec, o bé realitzar revisions sistemàtiques que tinguin en compte la qualitat metodològica dels estudis. Background: chronic wounds are those that are characterized by a prolonged evolution over time as a result of stagnation at some stage of the healing process, so that the anatomical or functional integrity of the lesional tissue is not recovered. Thanks to advances in modern medicine, the life expectancy of the population has increased, which has led to an aging population and proportionally has also increased the chronicity of wounds due to the fragility of the skin, weakness of the immune and endocrine system typical of the elderly population. Moreover, the abuse of antimicrobials has increased the appearance of resistant bacteria which further promote stagnation in the healing process due to the appearance of non-viable tissue. In relation with this stagnation in wound healing, it has been concluded that more research is needed and alternative treatments such as honey do not have enough research. Objectives: the purpose of this systematized review is to know the use of honey in the wound healing, especially those more chronic, like pressure ulcer, varicose ulcer, diabetic food and leg ulcer. Search methods: in order to create this review it has been used different kinds of research strategies to not loss crucial information about the topical use of honey in wounds. All the research strategies have been applied in the follow databases: Pubmed, CINHAL, Scopus, Cuiden Plus, COCHRANE, LILACS and Scielo. This process has been taking place from December 2020 to January 2021. Conclusions: Nonetheless the growing interest in using honey as an alternative in wound healing, the use of it in the reality is still insignificant. Honey stands out due to its properties, with its main ingredient is sugar, which give it a viscous texture and high osmolarity, and as well as its acid pH, give it the antimicrobial properties. Honey is more efficient rather than traditional methods as it gets bigger rates of wound healed and decrease wound healing time. In addition to this, it seems that honey also decrease pain and smell. For that reason, it is believed to be a fantastic alternative to prevent amputation in the diabetic foot. Related to pressure ulcer, it is thought to be a better product as well. However, with regards to varicose ulcers, the use of honey with compression does not showimportant results. By contrast, more research should be done with more participants and apply double-blinded. In the case of systematic reviews, methodological quality should be required.
