The diffusely enlarged uterus is a common finding in everyday clinical practice, however the management of such finding is not always a simple task. For the evaluation of the enlarged uterus both ultrasound scan and more recently ��RI have been used. The objective of this study is to evaluate quantitatively the reliability of the MRI in terms of providing a preoperative assessment of the histologic nature of the enlargement. This prospective study was conducted in the setting of the 1st Academic Department of Obstetrics and Gynecology of the Aristotele University of Thessaloniki from 2005 until 2009. Women with enlarged uteri who presented at the outpatient department and met the inclusion criteria were offered the option of participating. Those who accepted were subjected to MRI and subsequently to hysterectomy. The MRI diagnosis was then compared to the histopathology in order to evaluate the reliability of the former method in the correct preoperative assessment of the histologic nature of the enlargement. The study included in total 139 women, with ages ranging from 39 to 85 years, with a mean of 50.1 years. For the study of the agreement between the MRI and the gold standard method of histology Cramer���s V and Cohen���s kappa coefficients were used. Their values were calculated 0.672 and 0.610 respectively with standard error for the later 0.07. The sensitivity of the MRI for leiomyomas, combination of leiomyomas and adenomyosis, malignancies and other less common pathologies was calculated. The sensitivity for the above mentioned conditions was 96.9%, 38.1%, 66.6% and 80% respectively (p=0,05). The MRI according to Cramer���s V and Cohen���s kappa coefficients shows substantial agreement with histology. Moreover it is a very sensitive means of detecting leiomyomas (96.9%), while it is fairly sensitive for diagnosing malignancies (66.6%) and other pathology (80%). In the case of coexisting adenomyosis and leiomyomas the sensitivity observed was low (38.1%).
