Approximately half of the adolescents complaining of refractory dysmenorrhea or chronic pelvic pain will be diagnosed with endometriosis. The most frequent presenting symptom is pelvic pain, and the most common sign is pelvic tenderness. Treatment options include medical therapy such as prostaglandin synthetase inhibitors, oral contraceptives, progestins, danazol, or gonadotropin releasing hormone agonists; surgical options include laparoscopy using either cautery or laser, or laparotomy with conservative surgery. Most patients treated surgically are found to have minimal or mild disease, most frequently located in the cul-de-sac. Although endometriosis is thought to be a progressive disease, little is known about the long-term effectiveness of early treatment or the future fertility of these patients.
