Transgender women living with HIV (TGW) are reported to have lower rates of ART use and viral suppression, but STI incidence among this population has not been well described. We assessed HIV viral suppression and STI incidence rates as well as STI follow up care among HIV-infected TGW and matched controls. This retrospective cohort study at an academic center included 44 TGW, and age and race matched cohorts of 132 cis-gender women (CGW) and 132 cis-gender men who have sex with men (CGMSM), all living with HIV. We reviewed clinical records from 2011-2016 to gather demographics, HIV viral loads, and STI testing and follow up care. Binomial proportions with exact 95% confidence intervals were used to summarize rates for each group. Differences between groups were evaluated using logistic regression. All groups had similar rates of achieving and maintaining HIV virologic suppression. Compared to CGW, TGW and CGMSM were significantly more likely to have any incident STI (OR (95% CI): CGMSM vs CGW 3.74 (2.03, 6.88); TGW vs CGW 3.62 (1.64, 7.98)). All three groups had low rates of STI follow up testing with TGW having significantly lower rates of retesting (TGW 38.4%, CGMSM 64.3%, CGW 60.0%, p=0.03). All groups had similar rates of failure to retest due to providers not ordering STI testing at subsequent visits (TGW 38.9%, CGMSM 50.0%, CGW 48.4%). High incidence of STIs in TGW and CGMSM mirrors national trends and warrants increased research of underlying mechanisms and best practices to decrease rates. All groups had low STI follow up testing which is recommended by CDC STD treatment guidelines. Provider failure to retest patients in all groups was a leading reason for low follow up testing rates. Increasing awareness of high STI incidence in TGW could improve health outcomes among TGW living with HIV.
