Hensikt: Formålet med denne studien er å undersøke i hvilken grad kritisk syke pasienter som ligger på en intensiv avdeling får en adekvat piperacillin-behandling basert på behandlingsmål i relevante studier. Samtidig vil studien bidra til å bedre forståelsen av farmakokinetiske endringer av piperacillin hos kritisk syke pasienter. Dette skal nås gjennom måling av piperacillin-konsentrasjoner i plasma- og dialysatprøver. Pasientdata samles inn fra pasientjournal og legemiddelkurve for å kartlegge kovarianter som kan beskrive farmakokinetikken av piperacillin. Basert på disse dataene skal de farmakokinetiske parameterne (Vd, CL, t1/2, fT>MIC) estimeres ved hjelp av en farmakokinetisk populasjonsmodell. Ut fra dette vil det være mulig å undersøke sammenheng mellom farmakokinetiske parameterne av piperacillin og innsamlet pasientdata samt oppnåelse av behandlingsmålene. Problemstilling: Risikoen for underdosering av piperacillin er større hos kritisk syke pasienter enn andre pasientgrupper på grunn av endret farmakokinetikk hos kritisk syke pasienter. For lave plasmakonsentrasjoner av piperacillin kan føre til økt dødelighet hos disse pasientene. Kritisk syke pasienter som får kontinuerlig nyreerstattende behandling (CRRT) kan være særlig utsatt for underdosering. Dette skyldes at vannløselige legemidler som piperacillin elimineres ved CRRT. Metode: Studien ble uført som en prospektiv observasjonsstudie på Oslo Universitetssykehus, Ullevål ved avdelingene Generell Intensiv og Nevrointensiv i perioden fra august 2017 til mai 2018. Studien inkluderte pasienter eldre enn 18 år fordelt på to grupper avhengig av om de hadde akutt nyresvikt eller ikke. Pasientene med akutt nyresvikt fikk CRRT. Pasienter som hadde akutt eller kronisk nyresvikt uten behov for CRRT var ikke en del av studien. Studieperioden ble definert som første 72 timer etter oppstart med piperacillin/tazobactam (PIPTAZ). Piperacillin/tazobactam ble gitt som infusjoner over 30 minutter under hele studieforløpet. Dosering var 4/0,5 g piperacillin/tazobactam hver 6. eller 8. time. Plasma- og dialysatprøver ble tatt 24, 48 og 72 timer etter behandlingsoppstart. Farmakokinetiske parameterne av piperacillin ble estimert ved hjelp av en farmakokinetisk populasjonsmodell og resultatene ble analysert for å finne signifikante kovarianter. Behandlingsmålene ble definert som 100% fT>16mg/L og ≥50% fT>64mg/L. Resultater: 22 pasienter var inkludert i studien, hvorav 4 pasienter fikk CRRT. Studien inkluderte 64% menn og middelverdi for alder var 54 år. De fleste pasienter hadde traume (41%), postoperative komplikasjoner (27%) og septikemi (18%) som innleggelsesårsak. Det ble funnet signifikant korrelasjon mellom kreatinin-clearance (CrCL) og piperacillin-clearance, mellom pasientenes høyde og piperacillin-clearance og mellom plasma-albumin og halveringstid av piperacillin. Ingen av pasientene fikk behandling som nådde behandlingsmålene 100% fT>16mg/L og ≥50% fT>64mg/L under hele oppfølgingsperioden. Pasienter med CrCL ≥ 130mL/min (augmented renal clearance - ARC) fikk behandlingen som oppnådde lavest fT>16mg/L og fT>64mg/L, 35% og 12% i alle målingene for fT>16mg/L og fT>64mg/L. Den høyeste forekomsten av CrCL ≥ 130mL/min ble funnet hos pasienter med traume, sepsis og postoperative komplikasjoner som innleggelsesårsak. Konklusjon: Studien indikerer at dagens retningslinjer for bruk av PIPTAZ hos kritisk syke pasienter er utilstrekkelig og at det behøves endringer i doseringsregimene. Det anbefales bruk av terapeutisk legemiddelmonitorering (TDM) som et verktøy for dosejustering hvor PIPTAZ kan administreres oftere, over lengre tid eller som kontinuerlig infusjon, spesielt hos pasienter med økt CrCL. Tidligere studier bekrefter høyere grad av oppnåelse av behandlingsmål ved justering av dose basert på TDM. Pasienter med CrCL ≥ 130mL/min (ARC) er en spesielt sårbar pasientgruppe med stor risiko for å bli underdosert. Det anbefales daglig monitorering av CrCL hos alle kritisk syke pasienter som får behandling med PIPTAZ. I stedet for å måle CrCL fra 24-timers døgnurin kan det vurderes å måle CrCL fra 2, 8 eller 12 -timers urin. Pasienter med traume, sepsis og postoperative komplikasjoner som innleggelsesårsak er spesielt sårbare for å utvikle ARC og må derfor følges nøye opp i forhold til CrCL. Denne studien gir samtidig grunnlag for at det trenges videre forskning for å kunne fastsette retningslinjer for dosering av PIPTAZ hos kritisk syke pasienter. ; Aim of the study: The purpose of this study is to examine the extent of adequate piperacillin treatment based on treatment goals in relevant studies in critically ill patients admitted to an intensive care unit. The study should also contribute to and give us a better understanding of changes in pharmacokinetics of piperacillin in critically ill patients. This will be examined by measuring piperacillin concentrations in plasma and dialysate samples. Patients’ data that consists of laboratory test results and patient medical records will be used to map co-variables that can describe the pharmacokinetics of piperacillin. Based on these, the pharmacokinetic parameters (Vd, CL, t1/2, fT> MIC) will be estimated by using a population pharmacokinetic model. Based on this, it will be possible to investigate the relationship between the pharmacokinetic parameters of piperacillin and collected patients’ data, as well as the achievement of the treatment goals. Hypothesis: The risk for underdosage of piperacillin is higher in critically ill patients than in other patient groups as a result of altered pharmacokinetics in these patients. Too low plasma concentrations of piperacillin can lead to increased mortality in these patient group. Critically ill patients undergoing continuous renal replacement therapy (CRRT) may be particularly prone to underdosage. This is because water-soluble drugs such as piperacillin are eliminated by continuous renal replacement therapy. Materials and methods: The study was conducted as a prospective observational study in General Intensive and Neuro-intensive department at Oslo University Hospital, Ullevål hospital over a period from August 2017 to May 2018. The study included patients over 18 years of age assigned to two groups based on whether they suffered from acute renal failure or not. Patients with acute renal failure underwent CRRT. Patients with acute or chronic renal failure who didn`t need CRRT were not a subject of the study. Study period was defined as the first 72 hours after starting therapy with piperacillin/tazobactam (PIPTAZ). During the whole course of the study piperacillin/tazobactam was administered as 30-minute infusion. Dosage was 4/0,5 g piperacillin/tazobactam every 6 or 8 hours. Plasma and dialysate samples were taken 24, 48 and 72 hours after the start of the treatment. Pharmacokinetic parameters were estimated using a population pharmacokinetic model and results were analyzed to find significant co-variables. Goals of treatment were defined as 100% fT>16mg/L and ≥ 50% fT>64mg/L. Results: 22 patients were included in the study, 4 of whom underwent CRRT. The study included 64% men. The mean value for age was 54 years. The majority suffered from trauma (41%), postoperative complications (27%) and septicemia (18%) as reason for admission. Significant correlation was found between creatinine clearance (CrCL) and piperacillin clearance, as well as between height and piperacillin clearance and between plasma albumin and elimination half-life of piperacillin. None of the patients received a treatment that resulted in reaching the treatment goals 100% fT>16mg/L and ≥ 50% fT>64mg/L over the entire follow-up period. Patients with CrCL ≥ 130 mL/min (augmented renal clearance - ARC) received the treatment that reached lowest fT>16mg/L and fT>64mg/L, 35% and 12% for all measurements respectively. The highest incidence of CrCL ≥ 130mL/min was in patients who suffered from trauma, sepsis and postoperative complications upon admission. Conclusion: The study indicates that present guidelines for use of PIPTAZ in critically ill patients are inadequate and need changes in dosage regimen. The use of therapeutic drug monitoring (TDM) is recommended as a tool for dose adjustments so PIPTAZ could be administered more often, for a longer period of time or as continuous infusion, especially in patients with increased CrCL. Earlier studies confirmed a higher degree of achievement of treatment goals by adjusting dose based on TDM. Patients with CrCL ≥ 130mL/min (ARC) are a very vulnerable group with a high risk for underdosage. Daily monitoring of CrCL in all critically ill patients receiving PIPTAZ is recommended. It can be considered to measure CrCL from 2, 8 or 12 hours urine, instead of measuring CrCL from 24 hours urine. Patients suffering from trauma, sepsis and postoperative complications as reason for admission are particularly prone to developing ARC and must therefore be closely monitored for changes in CrCL. This study also provides the fundamentals for further research in order to establish guidelines for dosing PIPTAZ in critically ill patients.
