Background: The availability of smartphone apps for mental health problems has grown significantly over the past decade. Yet, there is limited research on the acceptability and feasibility of a smartphone app adjunct to psychological therapies for adolescents with emerging BPD symptoms, including self-harm and suicidal behaviours. Objective: The current study evaluated the acceptability and feasibility of integrating the DBT Coach app as an adjunct to the DBT skills group component of a comprehensive DBT programme for adolescents. The study also explored whether the DBT Coach app increases DBT skills and is related to changes in emerging BPD symptoms and other clinical outcomes by the end of the skills group, as an early indication of effectiveness. Method: A pre-post design was conducted using a mixed-method approach. DBT Coach was installed on participants' smartphones, and clinicians had access to the clinicians' portal as an adjunct to the DBT skills group for up to six months. Results: In total, 15 adolescents and seven clinicians intended to participate in the study, and 13 adolescents and six clinicians completed the study. Results indicated good acceptability and feasibility of the DBT Coach app and portal as an adjunct to the DBT skills group. A three-stage multiple hierarchical regression analysis revealed a significant positive relationship between app use and DBT skills practice. All other outcomes reported non-significant relationships. Thematic analysis generated four overarching themes and ten subthemes. Conclusion: This study is the first to examine the use of the DBT Coach app as an adjunct to the DBT skills group for adolescents with a history of self-harm and suicidal behaviours. The clinical and research implications of this study are discussed.
