Lymphome non hodgkinien simulant un carcinome rénal : A propos d'un cas de lymphome folliculaire / Non-Hodgkin lymphoma mimicking renal carcinoma : a case of follicular lymphoma

L'atteinte rénale est fréquente lors de la phase de dissémination des lymphomes non-hodgkiniens (LNH). En revanche, les LNH primitifs rénaux sont relativement rares. Ils peuvent être unilatéraux ou bilatéraux. Les auteurs rapportent un cas de lymphome folliculaire de localisation rénale unilatérale prédominante, simulant un carcinome rénal. Vingt-six autres cas de LNH primitifs ou apparemment primitifs rénaux unilatéraux ont été rapportés depuis 1980. L'âge moyen au diagnostic est de 64 ans avec une prédominance masculine. La clinique et l'imagerie médicale évoquent généralement un diagnostic de carcinome rénal. Il s'agit dans la majorité des cas d'un lymphome de grande malignité de phénotype B. L'existence de lymphomes, qu'ils soient primitifs rénaux ou ganglionnaires à localisation rénale prédominante, pouvant simuler un carcinome rénal doit être connue. En l'absence de diagnostic préopératoire, l'association néphrectomie-chimiothérapie entraîne le plus souvent des rémissions complètes durables.
