Lèpre tuberculoïde et spondylodiscite cervicale / Tuberculoid leprosy and pyogenic cervical discitis in the same patient

La lèpre est rare en Europe et le diagnostic en est délicat. Les auteurs rapportent une observation de lèpre tuberculoïde associée à une spondylodiscite à pyogènes. Le patient, âgé de 68 ans, d'origine antillaise, a été hospitalisé pour des cervicalgies inflammatoires. Le diagnostic de spondylodiscite C6-C7 à staphylocoque doré était rapidement retenu, justifiant une antibiothérapie. Toutefois, quelques aspects cliniques atypiques ont fait évoquer la présence d'une ostéoarthropathie nerveuse. La biopsie nerveuse révélait une atteinte axonale, mutilante, du nerf sensitif évoquant une lèpre. L'ensemble du tableau militait pour une lèpre dans sa forme tuberculoïde. Différents éléments font discuter le fait que la spondylodiscite cervicale a probablement été favorisée par l'existence d'une lèpre tuberculoïde évoluant depuis de nombreuses années.
