• Près de la moitié des patienta atteinta d'un lymphome non hodgidnien (LNH) sont âgés de plus de 60 ans au diagnostic et un tiers d'entre eux sont âgés de plus de 75 ans. Les LNH de type B diffus à grandes cellules représentent le sous-type majoritaire dans cette population. Un âge civil avancé ne constitue pas en soi un critère devant conduire à un traitement a posologies réduites et potentiellement sous-optimales. Dans ce contexte, l'évaluation des comorbidités joue un rôle majeur, la toxicité hématologique - avec le risque infectieux qui en découle - étant la principale toxicité limitante. En dehors d'une contre- indication cardiovasculaire formelle, l'utilisation d'anthracyclines dans le cadre d'une polychimiothérapie de type CHOP (cyclophosphamide, doxorubicine, vincristine, prednisone) constitue un standard thérapeutique. L'association du rituximab à la chimiothérapie a montré son intérêt à la fois dans les formes indolentes et dans les formes agressives de lymphomes de phénotype B du sujet âgé. Ce type de traitement est également réalisable chez certaina patients agés de plus de 80 ans.
