Nous rapportons un cas d'hépatite aiguë grave au paracétamol chez un enfant de cinq ans, hospitalisé pour un ictère, apparu après la prise de paracétamol à la dose de 100 mg/kg par jour pendant une semaine. Le diagnostic d'hépatite aiguë grave au paracétamol a été retenu sur la base des critères chronologiques, de la récidive des signes d'hépatite à la réadministration de paracétamol ; de l'absence d'autres causes et du score d'imputabilité à 10.
