O aumento da esperança média de vida aliado à inovação tecnológica, à maior facilidade de acesso a informação e exigência dos utentes desafiam, fortemente, o setor da saúde a ser mais eficaz e eficiente a prestar cuidados de qualidade aos utilizadores e de forma sustentável. A realidade vivida nas últimas décadas revela uma maior procura pelo setor hospitalar que se traduz num aumento da despesa na área da saúde agravado, naturalmente, pela pandemia da Covid-19. A tendência de aumento da despesa coloca em primeiro plano a preocupação com a sustentabilidade do sistema público de saúde, tornando-se inadiável a adoção de novos modelos de gestão que contemplem a melhoria dos serviços e a racionalização de custos focando-se não só nos serviços clínicos e na prestação direta dos cuidados de saúde, mas também na otimização da prestação dos serviços não-clínicos, no qual se insere a gestão da rouparia hospitalar. O presente estudo foi desenvolvido no Centro Hospitalar do Tâmega e Sousa, nomeadamente, no Hospital Padre Américo em Penafiel, e incidiu na implementação de soluções que permitiram melhorar a eficácia e eficiência dos processos integrados na gestão da rouparia hospitalar, o que se traduz em soluções mais sustentáveis. O acompanhamento, no terreno, do funcionamento do serviço e das práticas adotadas pelo hospital permitiu mapear o processo e identificar como pontos críticos: a falta de stock de roupa nos serviços; a falta de fardamento para os colaboradores; a localização inadequada da rouparia; a inexistência de rotas; a falta de controlo e normalização dos processos; a ineficiência dos procedimentos; o retrabalho na execução das tarefas; as movimentações excessivas, a desorganização e desarrumação das áreas de trabalho e o método de controlo das quantidades de roupa limpa e suja, que era efetuado através da pesagem dos carros. Para colmatar as falhas detetas foram desenhadas e implementadas soluções que compreenderam: a relocalização e reestruturação do layout da rouparia; a implementação de armazéns avançados em cada um dos serviços, a definição de rotas de entrega de roupa aos serviços e a instalação física de uma central de fardamento. Foi também desenvolvida uma aplicação que permite o registo das contagens efetuadas em cada serviço por tipologia de roupa, permitindo conhecer, em tempo real, o stock existente em cada serviço e o número de peças repostas. Realizaram-se dois inquéritos de satisfação para avaliar a qualidade do serviço prestado na rouparia, designadamente na reposição de roupa limpa aos serviços, na recolha de roupa suja e na central de fardamento. Para otimizar o controlo das quantidades de roupa, fez-se a monitorização dos armazéns avançados, comparando a contagem diária das tipologias de roupa suja dos serviços, com o stock definido. E a contabilização diária das peças de roupa limpa existentes, em stock, nos serviços, e da reposição efetuada. No total foram contados 6289 itens de roupa suja e 17907 peças de roupa limpa. Os dados recolhidos foram organizados e comparados com o stock definido para cada serviço. Da análise aos resultados obtidos, constata-se que é necessário reajustar as quantidades definidas para cada serviço, uma vez que se verificou que os consumos dos serviços são, maioritariamente, inferiores ao definido. Uma definição mais rigorosa permitirá uma melhor gestão dos stocks e, consequentemente, uma maior poupança.
The increase in the average life expectancy, the technological innovation and the access to a greater amount of information have resulted in new challenges for the health sector that is now being forced to be more efficient and effective when providing quality health care, in a sustainable way. The reality lived during the past few years reveals that there has been a greater demand for hospital services which translates into an increase in health services expenditure, which is, naturally, being worsen by the covid-19 pandemic we are currently facing. The tendency of increased expenditure puts the concern for the public health system´s sustainability at the fore. Thus, it is crucial that we adopt new management models that include the improvement of our services and the rationalization of costs. These models must not only focus on clinical services and the direct provision of health care but also on the optimization of the nonclinical services´provision, including the hospital linen management. The present study has been developed at Centro Hospitalar do Tâmega e Sousa, namely, at Hospital Padre Américo in Penafiel, and it has focused on the implementation of solutions that allow the improvement of the effectiveness and efficiency of the processes integrated into the hospital laundry management, which translates into more sustainable solutions. Monitoring, on the ground, the functioning of the service and the practices adopted by this hospital, has allowed the mapping of the process and the identification of critical points: the lack of clothing stocks in the services; the lack of uniforms for the working staff; the inadequate location of the laundry and linen room; the fact that there were no routes established; the lack of control and normalization of processes, the inefficiency of procedures, the rework in the execution of tasks, the excessive movements; the disorganization of work areas and the method used to control the amounts of clean and dirty clothes that was done by weighting the cars. In order to solve the problems detected, many strategies have been created and implemented: the relocation and restructuring of the linen room´s layout; the implementation of advanced warehouses in each service; the establishment of routes responsible for the delivery of clothing to the services and the physical installation of an uniform centre. Besides this, an app was developed, allowing the registration of every count made in each service, namely the count, accordingly to the type of clothing, of the existent stock in each service, as well as the number of replaced pieces. In addition, two enquires of satisfaction were made in order to evaluate the quality of the services being done by the laundry and linen team, namely the reposition of cleaning clothing in each service, the collection of dirty clothing and the way the central of uniforms was working. With the goal of optimizing the control of the amount of clothing, the advanced warehouses were monitored, through the comparison of daily counts of the dirty clothes services´ typologies to the previously defined stock. In total 6289 dirty clothing items were counted as well as 17907 items of clean clothing. The obtained data was organized and compared to the stock defined for each service. Analysing these results, it is concluded that a readjustment in the amounts defined for these services needs to be done. This happens due to the fact that the services consumption is, generally, inferior to the one previously established. Establishing more rigorous numbers will allow a better management of stocks which will result in greater savings.
