A reativação do vírus citomegálico humano (HCMV) em doentes ventilados em unidades de cuidados intensivos (UCI) e em estado crítico, tem sido considerada um fator de prolongamento do tempo de internamento na UCI, da duração da ventilação mecânica e do aumento da mortalidade e morbidade, associada a um pior desfecho clínico. Para esta investigação foram analisadas amostras de lavados bronco alveolares (LBA), referentes a 39 doentes ventilados internados na UCI do Hospital São Francisco Xavier (HSFX), 10 (25,6%) dos quais apresentaram reativação para o HCMV durante o internamento. A deteção do DNA do HCMV foi realizada pela técnica de PCR em tempo real in house (PCRtr). Não foram encontradas relações entre a reativação do HCMV e o sexo (p=1), idade (p=0,582), tipo de infeção (p=0,290), infeção por SARS-CoV-2 (0,074), tempo de entubação (p=0,097), tempo de internamento na UCI (p=0,111), tempo de internamento hospitalar (p=0,402) e morte do doente (p=0,590). Contudo, o desenvolvimento de infeções bacterianas durante o internamento na UCI foi associado ao aumento do risco de reativação do HCMV (p=0,027). Foram analisadas as concentrações das citocinas IL-1β e IL-8 através da técnica imunoenzimática de ELISA (RayBio® Human IL-1 beta/IL-8 ELISA Kit), tendo em conta os diferentes grupos de doentes com e sem reativação e que apresentaram ou não crescimento bacteriano. Houve diferenças significativas entre as concentrações das citocinas IL-1β e IL-8, (p=0,000) e (p=0,001) respetivamente, nos diferentes grupos. Verificou-se que amostras diferentes do mesmo doente apresentaram concentrações superiores quando de uma amostra foi positiva para DNA de HCMV (≥150 UI/mL), por comparação com uma amostra negativa para DNA do HCMV (<150 UI/mL). Não foi encontrada relação entre a carga viral e a evolução clínica dos doentes. Foram encontradas diferenças estatísticas significativas nas concentrações da citocina IL-1β dos doentes com a carga viral < 500 UI/mL em relação aos doentes com carga viral > 500 UI/mL. No entanto, não foram encontradas diferenças estatisticamente significativas nas concentrações da citocina IL-8 dos doentes com a carga viral < 500 UI/mL e carga viral > 500 UI/mL. Apesar da baixa amostragem ter condicionado os resultados, o presente estudo confirmou que a reativação do HCMV é comum nos doentes ventilados internados na UCI com infeções respiratórias.
The reactivation of the human cytomegalovirus (HCMV) in ventilated patients admitted to the intensive care unit (ICU) in critical condition, has been considered a factor for prolonging the length of stay in the ICU, duration of mechanical ventilation and increased mortality and morbidity, associated with a worse clinical outcome. For this investigation, samples of bronchoalveolar lavage (BAL) were analysed, referring to 39 ventilated patients admitted to the ICU of Hospital São Francisco Xavier (HSFX), 10 (25.6%) of whom showed reactivation for HCMV during hospitalization. The detection of HCMV DNA was performed using the in-house real-time PCR (trPCR) technique. No relationship was found between HCMV reactivation and sex (p=1), age (p=0.582), type of infection (p=0.290), SARS-CoV-2 infection (0.074), time of intubation (p =0.097), length of stay in the ICU (p=0.111), length of hospital stay (p=0.402) and death of the patient (p=0.590). However, the development of secondary bacterial infections during the ICU stay is associated with the risk of HCMV reactivation (p=0.027). The concentrations of IL-1β and IL-8 cytokines were analyzed using the immunoenzymatic ELISA technique (RayBio® Human IL-1 beta/IL-8 ELISA Kit), taking into account the different groups of patients with and without reactivation and who presented or no bacterial growth. There were significant differences between the concentrations of the cytokines IL-1β and IL-8, (p=0.000) and (p=0.001) respectively, in the different groups. The concentrations of cytokines from different samples from the same patient, showed higher concentrations in a positive sample for HCMV DNA ≥150 IU/mL, compared to a negative sample for HCMV DNA <150 IU/mL. No relationship was found between viral load and the clinical course of patients. Significant statistical differences were found in the concentrations of the cytokine IL-1β in patients with a viral load < 500 IU/mL compared to patients with a viral load > 500 IU/mL. However, no significant differences were found in the IL-8 cytokine concentrations of patients with viral load < 500 IU/mL and viral load > 500 IU/mL. Although the small sample size conditioned the results, the present study confirmed that HCMV reactivation is common in ventilated patients admitted to the ICU with respiratory infections.
